The Utah Health Insurance Marketplace collaborates with authorized partner Health Sherpa to streamline the enrollment process, ensuring residents have access to a user-friendly platform for selecting and enrolling in health insurance plans.

What You’ll Need Before You Start

Before you begin the enrollment process, gather the following documents and information. Having everything ready will help ensure your application goes smoothly and prevent delays:

Basic Personal Information

Full name, date of birth, and address.

Social Security number (or proof of legal residency, if applicable).

Phone number and email address for contact purposes.

Income Information

Recent pay stubs or proof of income (e.g., tax returns, W-2 forms, or other documentation of household income).

If self-employed, your most recent tax return and profit/loss statement.

Information about any unemployment benefits, alimony, or child support you may be receiving.

Use this information to estimate your income for the plan year.

Tax Information

Your most recent tax return or information on any tax exemptions, credits, or deductions that apply.

If you’re applying for financial assistance, you may be asked to provide your household’s Modified Adjusted Gross Income (MAGI).

Household Information

Names, birthdates, and Social Security numbers of all household members you want to include in the plan.

Information about whether any household members have access to employer-sponsored health insurance.

Current Health Insurance Details (if applicable)

Information about your current health insurance plan, including the provider, plan type, and coverage details, if you’re switching plans.

Citizenship or Immigration Status

Proof of U.S. citizenship or lawful immigration status for everyone applying for coverage.

This could include documents such as a passport, Green Card, or work visa.

Step-by-Step Enrollment Process

Once you have all your documents ready, follow these steps to enroll in a health insurance plan via HealthSherpa:

    • -Visit HealthSherpa and enter your Zip Code to find available health insurance plans in your area.
    • -Name (optional): Enter your full name to personalize your plan options.

    • -Phone Number (optional): Add your phone number if you’d like to receive direct support or follow-up about your health insurance options.
    • -Click See Plans and Pricing to move forward.
